CDI Medical Abbreviation: From Surgical Wounds to Chart Reviews

Credentials, teams and placesWound careCodingReviewed 30 Oct 2026

The CDI medical abbreviation has three common meanings: clean, dry and intact when describing a wound or dressing, clinical documentation integrity in hospital record review, and Clostridioides difficile infection in infection control notes. Which one applies depends on who wrote it and where.

Patients most often meet CDI after surgery, when a nurse checks an incision and writes that it is CDI, sometimes spelled C/D/I. Coding staff use CDI for a review program, and infection teams use it for a gut infection. This page shows each meaning in a real line and explains how to tell them apart.

CDI, C/D/I, c/d/i, or C.D.I. for the wound meaning
  • Where you see it

Post surgery nursing notes, coding reviews, infection reports
  • Used in

Surgery and wound care, health information management, infection prevention
  • Other forms

    I&D, incision and drainage

Three meanings of the CDI medical abbreviation

In a surgical or wound note, CDI is a quick status check: the incision or dressing is clean (no visible dirt or pus), dry (no fresh drainage soaking through) and intact (the edges are closed and the dressing is in place). It is a reassuring finding.

In hospital administration, CDI means clinical documentation integrity, also called clinical documentation improvement. Trained reviewers read charts and ask doctors to clarify notes so the record is accurate and complete.

In infection prevention, CDI stands for C. difficile infection, a bacterial infection of the colon that causes watery diarrhea and often follows antibiotic use. Context decides, and a nurse's wound note will never mean the infection.

The C/D/I medical abbreviation in surgery and wound notes

After an operation, nurses and surgeons check the incision at set times. If it looks healthy, they often write CDI and move on to the next item.

Sample wound lines that use CDI
Note linePlain meaning
Dressing C/D/IThe bandage is clean, dry and in place
Lap sites x4 CDI with skin glueFour small keyhole cuts look clean, dry and closed
POD 1, incision CDI, ambulatingOne day after the operation the wound looks healthy and the patient is up walking
Dressing not CDI, scant serous drainageA small amount of clear fluid has come through

Handoff at shift change: "Left hip incision CDI, pain 3/10, voiding." The outgoing nurse is telling the next nurse that the hip wound looks healthy, pain is mild and the patient is passing urine.

Clinical documentation integrity in coding

CDI programs sit between clinicians and coders. A CDI specialist, often a nurse or a clinician with coding training, reviews charts while the patient is in the hospital. If a note says only "low sodium," the specialist may send a query asking whether a more specific diagnosis fits. The doctor decides; the reviewer may not suggest an answer that the record does not support.

Accurate notes affect how the stay is coded with ICD codes, how the hospital is paid through systems such as DRG groups, and how quality data look to the public. Good documentation also helps the next clinician understand what happened.

C. difficile infection in infection control

When CDI appears in an infection report or a stool test result, it means Clostridioides difficile infection. The germ was formerly named Clostridium difficile, which is why some staff still say C. diff. It spreads through spores on hands and surfaces, and soap and water work better than alcohol gel against those spores. Patients with CDI are usually placed on contact precautions, and staff wear gowns and gloves in the room.

How to tell which CDI you are reading

Because this medical acronym does triple duty, a reader who meets CDI has to sort it out. Ask three questions. Is the sentence about a wound, dressing or incision? Then it means clean, dry and intact. Is it about a query, documentation or a coding review? Then it is the documentation program. Is it about diarrhea, a stool test, isolation or antibiotics? Then it is the infection. If none of these fit, check with the author. Records staff and hospital roles have their own abbreviation section too.

A wound that looks clean in a note can still need watching at home. Your surgical team or nurse can tell you what signs to report after you leave.

Frequently asked questions

What does CDI mean for a wound?

It means clean, dry and intact. The incision or dressing shows no fresh drainage, no obvious signs of infection, and the edges or bandage are in place.

What does C/D/I mean after surgery?

C/D/I is the same as CDI, clean, dry and intact. Nurses use it to show that a surgical incision or dressing looks healthy at the time of the check.

What is a CDI specialist?

A CDI specialist reviews hospital charts to make sure the documentation is complete and accurate. They send questions, called queries, to doctors when a note is unclear.

Is CDI the same as C. diff?

In infection control, CDI stands for C. difficile infection, which people often call C. diff. In wound and coding notes, CDI has different meanings, so check the setting.

Why do nurses write CDI instead of normal?

CDI tells the reader exactly which three things were checked. Writing normal alone would not say whether the dressing was dry or still in place.
