CKD Medical Abbreviation: Chronic Kidney Disease and Its Stages

Conditions and diagnosesNephrologyStaged by GFRReviewed 30 Oct 2026

The CKD medical abbreviation means chronic kidney disease, a lasting decrease in how well the kidneys filter the blood, or signs of kidney damage, present for three months or longer. It is usually written with a stage number, such as CKD 3a, that shows how much filtering ability remains.

CKD often causes no symptoms in its early stages, so many people first meet the abbreviation on a lab report or a problem list after a routine blood test. Diabetes and high blood pressure are the two most common causes. This guide explains how the stages work, how to read CKD in a note, and the related shorthand you will see beside it, such as GFR and ESRD.

At a glance

  • Stands for

    Chronic kidney disease
  • Written as

CKD, CKD 3a, CKD stage 4, CKD G3bA2
  • Used in

Nephrology, primary care, diabetes care, pharmacy
  • Where you see it

Problem lists, lab comments, medication dosing notes
  • Do not confuse with

AKI, a sudden kidney injury that may recover
  • Older term

    CRF, chronic renal failure

What CKD means in a medical record

Healthy kidneys filter waste and extra fluid out of the blood, balance salts, help control blood pressure and make hormones for red blood cells and bones. In chronic kidney disease, that work slowly declines. The word chronic is the key: the change has lasted at least three months, which separates it from a short term problem.

Doctors diagnose CKD from two kinds of evidence: a lower estimated glomerular filtration rate (eGFR) on a blood test, or signs of damage such as protein (albumin) in the urine, even if the eGFR is still normal.

CKD stages by filtration rate

Stages run from 1 to 5 based on eGFR, measured in milliliters per minute per 1.73 square meters of body surface. The numbers below are the standard cutoffs.

The five CKD stages at a glance
StageeGFR (about)Plain description
190 or aboveNormal filtering, but signs of damage
260 to 89Mild decrease with signs of damage
3a45 to 59Mild to moderate decrease
3b30 to 44Moderate to severe decrease
415 to 29Severe decrease
5Below 15Kidney failure

Many reports also add an albumin category, A1 to A3, for how much protein is in the urine. So CKD G3a A2 means stage 3a filtering with a moderate amount of urine albumin. Labs may present these results in their own format.

The CKD medical abbreviation in a real note

Primary care note: "DM2, HTN, CKD 3b. eGFR 38, stable from 40 last visit. UACR 120. Continue lisinopril, avoid NSAIDs, renally dose meds, refer nephrology."

Read in plain words: this person has type 2 diabetes, high blood pressure and stage 3b chronic kidney disease. The filtering rate has stayed about the same. A urine test shows some albumin. The plan keeps a blood pressure medicine that also protects the kidneys, avoids pain relievers such as ibuprofen that can strain the kidneys, adjusts other medicine doses for kidney function and sends the person to a kidney specialist.

CKD versus AKI, ESRD and other kidney shorthand

Kidney abbreviations that often appear together
AbbreviationMeaningHow it differs from CKD
AKIAcute kidney injurySudden, over hours or days, and may recover
ESRDEnd stage renal diseaseFunction so low that dialysis or a transplant is needed
BUNBlood urea nitrogenA lab value, not a diagnosis
PKDPolycystic kidney diseaseAn inherited cause of CKD

A note can say AKI on CKD, meaning a sudden worsening on top of long standing kidney disease.

Why pharmacists care about CKD

The kidneys clear many medicines from the body. When filtering drops, some drugs build up, so pharmacists check kidney function before dispensing medicines such as metformin, certain antibiotics and some blood thinners. A CKD label on the chart can prompt a dose change or a different choice. Pharmacy and nursing students learn to look for the latest eGFR or creatinine on the BMP when they see CKD.

Good questions to bring to a kidney visit

  • What stage is my kidney disease, and has it changed?
  • What is causing it, and what can slow it down?
  • Do any of my medicines or over the counter pain relievers need to change?
  • How often should my blood and urine be checked?

Frequently asked questions

What does CKD stand for on lab results?

CKD stands for chronic kidney disease. A lab comment may mention it when the eGFR has been low for some time, but the diagnosis is made by your clinician, not by one result.

Is CKD the same as kidney failure?

Not usually. Kidney failure is stage 5 of CKD. Most people with CKD are in earlier stages and still have meaningful kidney function.

What does CKD 3a mean?

It means the estimated filtering rate is about 45 to 59, a mild to moderate decrease. Stage 3 is split into 3a and 3b because risks differ across that range.

Can CKD get better?

Lost kidney function rarely comes back, but its progress can often be slowed. Blood pressure and blood sugar control and avoiding kidney stressing medicines are common parts of care.

What is the difference between CKD and AKI?

AKI is a sudden drop in kidney function over hours or days, often from dehydration, infection or a medicine. CKD lasts three months or more and changes slowly.
