OT Medical Abbreviation: Occupational Therapy Explained

Credentials, teams and placesRehabCare teamReviewed 30 Oct 2026

The OT medical abbreviation means occupational therapy, or the occupational therapist who provides it. OT helps people do the everyday tasks of life, such as dressing, bathing, cooking and getting back to work, after an illness, injury or surgery, or when living with a disability.

You will often see it paired with physical therapy as PT/OT in hospital orders. In surgical notes from the UK, India, Australia and other countries, OT has a second meaning: operating theatre, the room where surgery happens. The sections below explain both and show sample lines.

What occupational therapy covers

Occupation here means anything that fills your day, not only paid work. An occupational therapist looks at what a person needs and wants to do, finds what gets in the way, and builds a plan to close the gap. That can mean exercises for hand strength and coordination, practice with dressing or using the toilet safely, memory and thinking strategies, splints, or adaptive tools such as a long handled reacher or a shower chair.

OT also looks at the home and workplace. A therapist might suggest grab bars, a raised toilet seat or a new way to arrange the kitchen so a person can stay independent.

PT and OT: how the two therapies differ

PT/OT appears together so often that people assume they are the same. They work side by side but aim at different goals.

Physical therapy and occupational therapy side by side
PointPTOT
Main goalMovement, strength, walking, balanceIndependence in daily tasks
Typical sessionGait training, stairs, leg exercisesDressing, grooming, kitchen tasks, hand use
Common toolsWalkers, canes, exercise bandsReachers, splints, adaptive utensils

After a hip replacement, for example, PT works on walking, while OT teaches how to put on socks and shoes without bending too far.

Reading the OT medical abbreviation in hospital orders

Orders and notes use OT in a few standard phrases.

  • OT eval and treat: the therapist assesses the patient and starts therapy as needed.
  • OT consult: a request for the therapist to see the patient.
  • OT to follow: therapy will keep seeing the patient during the stay.
  • Home with OT: therapy will continue at home or in an outpatient clinic.

Therapy note: "OT: pt min assist for lower body dressing, recommend tub bench." Some hands on help was needed below the waist, and the therapist suggests a bench for safer bathing.

OT meaning operating theatre in surgery notes

In British English and in many countries that train doctors in that tradition, the room for surgery is the operating theatre, shortened to OT. A note such as taken to OT for laparotomy means the patient went to surgery. American notes use OR, operating room, for the same place.

If you are reading records from abroad, or a note written by a clinician trained overseas, keep this meaning in mind. Words like incision, anesthesia, theatre list or post op point to surgery rather than therapy.

OTR/L, COTA and other titles you may see

Occupational therapists in the US usually sign notes with OTR/L, which stands for occupational therapist, registered and licensed. An occupational therapy assistant may sign as COTA, certified occupational therapy assistant, and works under the therapist's plan. Both work with the wider rehab team, including speech therapy (SLP) and physical therapy.

Badge and signature titles beyond therapy are gathered in the credentials, teams and places section.

Questions to ask the therapy team

If OT is part of your plan or a family member's plan, a few questions can help.

  • What daily tasks are we working on first?
  • Will any equipment be needed at home, and who arranges it?
  • How many sessions are likely, and where will they happen after discharge?
  • Are there exercises to practice between visits?

OT is described here only as a term. Your therapist and care team can say what therapy plan fits your own situation.
