ARF Medical Abbreviation: Three Meanings and How to Tell Them Apart

Conditions and diagnosesHospitalSeveral meaningsReviewed 30 Oct 2026

The ARF medical abbreviation has three common meanings: acute renal failure, a sudden drop in kidney function; acute respiratory failure, when the lungs suddenly cannot keep oxygen or carbon dioxide at safe levels; and acute rheumatic fever, an inflammatory illness that can follow a strep throat infection.

Because these are very different problems, context decides, and many hospitals now encourage writers to use more specific terms. Kidney specialists prefer AKI, and lung teams often write AHRF or spell out the type. This page shows how to read each meaning and the clues that separate them.

No single meaning is safe to assume. The surrounding lab values, treatments and specialty almost always make it clear.

ARF as acute renal failure

For many years, ARF meant a sudden loss of kidney function. Kidney specialists now prefer acute kidney injury, AKI, because it covers milder rises in creatinine that still matter. You will still meet ARF in older charts and in some current notes.

Discharge summary: "Hospital course complicated by ARF, Cr peaked at 3.1, improved with IV fluids." Creatinine, a kidney blood test, rose and then came back down with fluids. The number is an example of format, and each lab sets its own range.

Kidney related notes also tend to show BUN, urine output and medicine adjustments.

ARF as acute respiratory failure

In emergency and ICU notes, ARF often means acute respiratory failure. It is commonly divided into two types. Hypoxemic failure means oxygen in the blood is too low. Hypercapnic failure means carbon dioxide is building up because breathing is not moving enough air. An arterial blood gas, ABG, helps sort the type.

ICU note: "ARF with hypoxemia 2/2 PNA, on HFNC." Here ARF is respiratory: low oxygen secondary to pneumonia, treated with high flow oxygen by nasal cannula.

Severe lung inflammation called ARDS is one cause of this kind of ARF.

ARF as acute rheumatic fever

Acute rheumatic fever is an immune reaction that can develop a few weeks after an untreated or undertreated strep throat. It can cause fever, painful joints that move from one to another, skin changes, jerky movements and inflammation of the heart. Its main lasting risk is rheumatic heart disease, which damages heart valves.

It is uncommon in many high income regions but still seen worldwide, mostly in children and young adults. Pediatric and cardiology notes are the usual home for this meaning, often with a mention of strep testing or valve findings on echo.

Clues that tell you which ARF is meant

Context clues for ARF
If the note mentionsARF likely means

Oxygen, ABG, ventilator, BiPAP, intubationAcute respiratory failure

When the note still leaves you unsure, ask the writer or the care team. Students should spell out the full term on first use, which many facilities expect for abbreviations with more than one meaning.

ARF and its look alike abbreviations

CRF can mean chronic renal failure, an older name for long term kidney disease now usually written CKD. AHRF names acute hypoxemic respiratory failure specifically. ARDS is a cause of respiratory failure, not a synonym for it. Grouped condition shorthand for kidneys, lungs and the heart sits in the diagnosis abbreviations guide.

This page explains general uses of an abbreviation. If ARF appears in your own records and the meaning is not clear, your nurse or doctor can tell you which condition was meant.

Frequently asked questions

What does ARF stand for in medical terms?

ARF most often means acute renal failure or acute respiratory failure, and less often acute rheumatic fever. The setting and the tests mentioned around it show which one is meant.

Is ARF the same as AKI?

When ARF means acute renal failure, AKI is the newer, preferred term for the same kind of sudden kidney problem. AKI also includes milder kidney injury.

What does ARF mean in the ICU?

In intensive care, ARF usually means acute respiratory failure, when the lungs cannot keep oxygen or carbon dioxide at safe levels without extra support.

Why is ARF considered a confusing abbreviation?

It stands for three unrelated conditions affecting the kidneys, lungs or heart and joints. Many hospitals encourage writing the full term or a more specific abbreviation to avoid mix ups.
