You will see the CLD medical abbreviation used for chronic liver disease, chronic lung disease and clear liquid diet. The first two are diagnoses, and the third is a diet order, so the place the letters appear usually makes the meaning plain.
A gastroenterology or hepatology note that lists cirrhosis or hepatitis is about the liver. A newborn intensive care chart is likely about the lungs of a premature baby. A diet line in hospital orders is about what a patient may eat and drink. The sections that follow take each reading in turn, with an order or note line to decode.

Which CLD is meant: a quick guide
| Meaning | Where you see it | Clues nearby |
|---|---|---|
| Chronic liver disease | GI, hepatology, internal medicine | Cirrhosis, hepatitis, ascites, LFTs |
| Chronic lung disease | Neonatal and pediatric lung care | Weeks of prematurity, oxygen needs |
| Clear liquid diet | Diet orders, procedure prep, after surgery | Diet:, advance as tolerated, NPO after midnight |
If the clues clash, a quick question to the author clears it up. Dietary staff, nurses and doctors may each be thinking of a different CLD.
CLD medical abbreviation in GI: chronic liver disease
Chronic liver disease means the liver has been injured over a long period, usually six months or more, leading to inflammation and scarring. Common causes include heavy alcohol use, written EtOH in many notes, long-term hepatitis B or C infection such as HCV, and fat buildup linked to metabolic health, now called MASLD. Autoimmune and inherited conditions are less common causes.
Over time, scarring can progress to cirrhosis. Charts may then mention ascites, which is fluid in the belly, varices, which are swollen veins, or confusion from toxins the liver is not clearing. Liver blood tests, imaging and sometimes a biopsy track how much damage is present.
Chronic lung disease in babies
In neonatal care, CLD usually means chronic lung disease of prematurity, which overlaps heavily with bronchopulmonary dysplasia, written BPD. It describes babies born early who still need extra oxygen or breathing support weeks after birth, commonly assessed at 36 weeks of corrected age. Their lungs were still developing when they needed ventilation and oxygen, which can injure the delicate tissue.
In adult notes, chronic lung disease can be a loose umbrella for long-term conditions such as COPD or interstitial lung disease, though more specific labels are usually preferred.
CLD medical abbreviation diet orders
As a diet order, CLD means a clear liquid diet: fluids you can see through at room temperature. It gives fluid, some salt and some sugar with almost no residue left in the gut. It is commonly ordered before a colonoscopy or certain tests, for a short time after some surgeries, or when someone is restarting food after vomiting or bowel rest.
- Usually allowed: water, clear broth, pulp-free light-colored fruit juice such as white grape, plain gelatin, ice pops without fruit pieces, tea or coffee without milk, clear sports drinks
- Usually not allowed: milk, juices with pulp, soups with pieces, anything solid
Some procedure preps also exclude red or purple colors. Hospitals print their own lists.
Sample CLD lines decoded
Post-op order: "Diet: CLD, advance to full liquids then soft as tolerated." The patient starts with clear liquids and moves to thicker foods if they do well. Full liquids add milk-based items such as pudding and cream soups.
Hepatology note: "CLD 2/2 EtOH with compensated cirrhosis, no ascites." The 2/2 means secondary to, so this reads as chronic liver disease caused by alcohol, with cirrhosis that is not yet causing major complications.
The order to have nothing by mouth is NPO, often the step before a clear liquid diet.
For students and new staff
Diet orders and diagnoses live in different parts of the chart, which is the easiest way to avoid a mix-up. Many hospitals prefer the full words clear liquids in diet orders. When charting, a specific diagnosis such as alcohol-related cirrhosis or bronchopulmonary dysplasia tells the next reader more than CLD alone.
Liver, lung and other problem list shorthand is gathered on the conditions hub page.
This is general information about the abbreviation. If you have been given a clear liquid diet or a liver or lung diagnosis, your care team or dietitian can explain exactly what applies to you.
Frequently asked questions
What does CLD mean on a diet order?
On a diet order, CLD means clear liquid diet. That allows see-through fluids such as water, clear broth, strained light juice and plain gelatin.
What does CLD mean in GI notes?
In gastroenterology and liver notes, CLD means chronic liver disease. It describes long-term liver injury from causes such as alcohol, hepatitis or fat buildup.
Is CLD the same as BPD in babies?
Neonatal teams frequently treat the labels as near twins for one picture: lasting lung trouble in a baby born early who still needs oxygen. BPD is the more specific name.
Is CLD the same as cirrhosis?
Not always. Cirrhosis is the advanced, heavily scarred stage of chronic liver disease, but CLD also covers earlier stages with less scarring.
Can I drink coffee on a clear liquid diet?
Many clear liquid lists allow black coffee or tea without milk or creamer. Your hospital or procedure instructions are the final word, since some preps add limits.
