HCC Medical Abbreviation: Liver Cancer, Coding and Context

Conditions and diagnosesTwo meaningsLiverReviewed 30 Oct 2026

The HCC medical abbreviation has two common meanings: in a clinical note it means hepatocellular carcinoma, the most common type of primary liver cancer, and in billing and coding it means Hierarchical Condition Category, part of the system Medicare and other payers use for risk adjustment. The setting tells you which meaning of this medical acronym is intended, in other words whether the HCC medical abbreviation names a diagnosis or a payment category.

A radiology report, liver clinic note or cancer care plan almost always means the liver cancer. A coding audit, a payer letter or a documentation tip sheet means the category system, often written CMS-HCC or HHS-HCC. Both are covered here, with clues for sorting one from the other.

HCC medical abbreviation for liver cancer

Break the clinical term into parts: hepato means liver, cellular refers to cells, and carcinoma names a cancer arising from the cells that line or make up an organ. Hepatocellular carcinoma begins in the hepatocytes, the main cells of the liver. It is different from cancer that spreads to the liver from somewhere else, such as the colon.

Most cases grow in a liver that is already scarred. Common background conditions include cirrhosis, long term hepatitis B or hepatitis C infection, heavy alcohol use, and fatty liver disease now called MASLD. For this reason, people with cirrhosis are often offered regular surveillance, usually a liver ultrasound about every six months, sometimes with a blood test called alpha fetoprotein (AFP).

How liver cancer HCC appears in reports

Because liver cancer can often be diagnosed from its typical look on contrast imaging, the abbreviation appears early in radiology language. A sample report line and its plain reading follow.

Plain reading of a liver imaging impression
Report wordingWhat it means
Cirrhotic liver morphologyThe liver has the lumpy shape of scarring
2.4 cm arterial enhancing lesion, segment 6A spot that lights up early with contrast, in one section of the liver
with washout and capsuleFeatures that strongly suggest liver cancer
Consistent with HCCThe findings fit hepatocellular carcinoma

Treatment options depend on tumor size, liver function and overall health. They can include surgery, liver transplant (OLT), ablation such as RFA, treatments delivered through the liver arteries, radiation, and medicines.

HCC in coding: Hierarchical Condition Category

On the payment and coding side, HCC means Hierarchical Condition Category. Payers group thousands of diagnosis codes into a smaller set of categories that predict how much care a person is likely to need. Each category carries a weight, and the weights add up to a risk score, often called a RAF (risk adjustment factor) score.

The CMS-HCC model is the version used for Medicare Advantage plans, run by the Centers for Medicare and Medicaid Services (CMS). The HHS-HCC model is a related version used for individual and small group plans in the health insurance marketplaces. Hierarchical means that when a person has a milder and a more severe form of the same condition, only the more severe category counts.

Why coders and clinicians talk about HCCs

Risk adjustment depends on chronic conditions being documented clearly in the medical record, usually at least once each calendar period from a qualifying visit. Coding and CDI teams often teach the MEAT check: the note should show the condition was Monitored, Evaluated, Assessed or Treated.

In a cardiology visit note: "Chronic systolic heart failure, EF 30 percent, stable on current meds, continue plan." A coder would see that the heart failure was assessed and treated, so the diagnosis supports an HCC. This is why the HCC medical abbreviation comes up so often in cardiology and in kidney care and diabetes clinics, where many qualifying chronic conditions live.

Common condition groups that map to HCCs include diabetes with complications, heart failure, CKD stages, COPD, some cancers and major depression.

Clinicians rarely use the coding meaning in patient care notes, and coders do not use it as a diagnosis. If the line mentions the liver at all, assume the cancer meaning. For more shorthand that sits on problem lists, the diagnosis abbreviations hub groups them by system.

Questions a patient might bring to the visit

  • Does my liver condition mean I should have regular imaging, and how often?
  • What stage is the tumor, and what treatment options fit my liver function?
  • Why is a diagnosis listed on my visit summary that we did not discuss today?

The last question often traces back to coding. Chronic conditions may be listed because the record needs to reflect them, and your clinic can explain why.

This page decodes two uses of the same letters. A liver cancer diagnosis, a screening plan or a question about your own record should be talked through with your hepatologist, oncologist or care team.

Frequently asked questions

What does HCC stand for in the liver?

It stands for hepatocellular carcinoma, the most common cancer that starts in the liver itself. It usually develops in a liver already damaged by cirrhosis or chronic hepatitis.

What does CMS-HCC mean?

CMS-HCC is the Hierarchical Condition Category risk adjustment model used by the Centers for Medicare and Medicaid Services for Medicare Advantage. It turns documented diagnoses into a risk score that adjusts payments to health plans.

What is the HHS-HCC model?

It is a related risk adjustment model used for individual and small group plans sold through the health insurance marketplaces. It works on the same idea of grouping diagnoses into weighted categories.

Is HCC the same as HCV?

No. HCV is the hepatitis C virus, an infection, while HCC in a medical chart is a liver cancer. Long term hepatitis C is one of the known risk factors for hepatocellular carcinoma.

Why is HCC on my problem list if I do not have cancer?

Usually it is not. Check the full wording, since the coding meaning almost never appears on a patient problem list. If the line truly reads HCC and you are unsure why, ask your clinician to explain.
